Residential accounts that remain unpaid more than twenty (20) days after the billing date, will be subject to a late payment charge of 1.5%. Business accounts will also be subject to a late payment charge of 1.5% with a minimum late payment charge of $5.00. Payments received after the 20th of the month will not appear on the following month's statement. The check will be used solely as a source document for capturing the routing number, account number, check serial number and dollar amount of the source document. The provision of this notice by State Long Distance Telephone Company to you and the receipt of the source document (your check) together constitute authorization of ARC entry (Accounts Receivable Entry) or bank draft. A fee in the amount of $19.50 may be assessed for ACH (Automatic Clearing House) debits returned due to non-sufficient funds or due to account closure.
